About 16-in-1 LNB you must configure the tuner in Advanced mode and configure every satellite you can receive with correct settings.
I think you should play with Committed/Uncommitted commands and DisecQ 1.1 addresses (AA,AB,BA,BB) and you can configure all your LNBs.

I've never configured this kind of complex satellite system ...

did EXACTLY as you have instructed ... but as soon as i turn ON the square button it goes back to VU+ TV Player logo ... loading (forever) !

should i factory reset?

no, format again your usb stick and be sure to format in fat32
or try another.

Make sure you have no other USB devices connected and try and flash from a rear USB port.

Unless I am mistaken with your setup they all need to be saved as LNB1 and you just use the committed DiSEqC command to change between then.

However it has been about 4 years since I used a DiSEqC switch myself and even then I only had 3 feeds not 16.

i dont have any clue about commited/uncommited stuff ... but are you sure about all save as LNB1?

i had this setup for 16/1 in DiSEqC 1.1 on Dr HD F15 receiver, worked flawlessly there.

Not 100% as I said it's been a long time since I used a switch, but it does sound right in my head.

To flash your Duo2, you need to make sure you have no usb devices plugged in, other than the freshly fat32 formatted usb stick you are using with the image on it.

If you have installed a hdd, that has previously been used with a vu+ box you will also need to make sure there are no backup files saved on it.

no other USB plugged. I have installed a fresh brand new 2.5 HDD in it today, anything to do about that?

Code:

Download and use this USB format tool.

1.Take the cheapest USB stick you have (1 or 2 Gb)
2.Format USB stick on your PC FORMAT FAT 32
2.UNZIP image file for DUO2 you have downloaded from the forum on your PC
3. When you UNZIP check if you have the following folders
vuplus and sub folder duo2 ( so you must have /vuplus/duo2 folder)
in duo2 folder you must have 5 files.
4. Now you can copy vuplus folder with all (/vuplus/duo2/all 5 files) on your USB
5.turn off box
6.Connects your USB stick into the USB slot/connector on the front
7.Switch box ON
8.press the button above the USB connector (above the USB stick)
9.Flash should start
10.When complete, remove the USB stick
Switch box ON/OFF again if you cannot start image with the remote (switch on)
